Acknowledgement and Agreement

By submitting this form, I acknowledge and agree to the following:

Billing Authorization:
I authorize Marian University to bill the amount requested above to my student account.
Payment Responsibility:
I understand that this charge is non-refundable and payment is required according to university billing policies.
Processing Time:
Approved funds will be added to my printing balance by the next business day after submission of this form
Annual Reset:
All printing accounts will reset to the standard $30 annual allowance on July 1 each year. Any unused balance will expire and be cleared on June 30 of that year.
